This just happened in the game(27055307) i'm watching.

2 4:30 38 — 46 E. Nève (H) collects the rebound and goes back up for the shot, guarded closely by K. Proot (A). Scored.
2 4:30 38 — 46 K. Proot (A) is called for a shooting foul.
2 4:30 38 — 46 M. Tyumentsev (H) substituted for K. Bultinck (H) at SG.
2 4:30 38 — 46 Change at PF: J. Sass (H) in. P. Coemelck (H) out.
2 4:30 38 — 46 Substitution at C: P. Moss (H) for J. Van Sumere (H).
2 4:30 38 — 46 D. Fraussen (A) substituted for R. Sturbelle (A) at SG.
2 4:30 38 — 46 S. N'Diaye (A) was replaced by J. Riga (A) at SF.
2 4:30 38 — 46 S. Nauwelaers (A) brought in for I. Choudhury (A) at PF.

Although this is from the Play-By-Play, the exact same information appeared in the match viewer.
Not sure which player actually got credited with the FTM/A but my team got the and-1 anyway.

Just a simple bug in the order of the substitution and the free throw.

Ok, but doesn't a team have to call a timeout after a made free-throw if they want to sub-out the player who got fouled - and had to stay on the floor to shoot the free-throw?
If he misses it, he most definitely has to wait for a stoppage to be subbed-out. But I believe even if he makes it and the other team has to inbound the ball, the free-throw shooting team can't make subs at that point.
