If I have 3 starters, and 1 backup, does anyone knows what happens?
specifically, will my 1 backup sub in for a fan and end up playing 48 minutes as well?

Yes, your sub will be a fourth starter and play 48 minutes.

Pho3niX is exactly right. The best way to accomplish your goal is buy a couple $1k players and play them at the two empty positions.
