Why are my minutes so imbalanced? I set the starter on the first column and a backup of the same position on the 2nd and 3rd column. My games are set to strictly follow depth chart and not bench when fouled.

Yet, my starters are getting 40+ minutes while my bench is not even getting 8 minutes.

How do I fix this?

It's probably because your starters are a lot better than your backups and the game was close. In close games, your coach wants the best players on the floor, and he will only substitute if their performance is lower because of fatigue. If they have high stamina, they'll remain better than your bench for a long time. You'll see that in games you're ahead by a big margin, your bench players will get more minutes, sometimes even 50/50.

This doesn't seem to be the case. For one, my small forwards are equally good (nearly at the same game shape and salary). Yet, this was the most imbalanced one because their minutes were 48 and 0.

What are my options? Should I but better players and overhauling my entire roster?

It's probably more the stamina of your starters is too high. It's unfortunate that the state of the game is such that "stamina too high" is a concept, but if your starters have respectable or higher stamina, it's hard to get them out of the game. If the backup is nearly as good, your best bet is just to alternate which one starts from game to game.

This might be true but not in my case. Besides the PF position, all of my bench have strong or higher stamina compared to my strong or lower starters. This is very strange.

The stamina on the bench players doesn't really matter. If you have respectable stamina, your starter will probably get over 40 minutes on average (excluding foul outs, injuries and garbage time).

You have high stamina players.
Exert that stamina. With high stamina players you can use attack offenses and defenses that will make the coach substitute at the time when it is not puzzling. ..a lot of people don't get that.
FCP...RNG.... have fun with it, and the substitution will even out. ....
Just an option.
