Luckily this has rarely happened to me, but is frustrating when it does happen. The opposing coach has pulled all his starters long ago and your team is down by 20+ points in fourth quarter and making no headway into shrinking your opponents lead. Shouldn't there be a point where your coach says "ok that's it get the starters off the floor." In reality coaches would take out their starters for fear of injury for a hopeless cause. Training is also screwed up because starters have been in the game for 45+ minutes trying to win the unwinnable.

My suggestion is for the losing team's starter's to be pulled if the team is down by 20+ in the final five or six minutes. Giving reserves some minutes who may have a better chance of winning then exhausted starters.

Which coaching strategy did you pick?
"let coach decide" might have a different result compared to "follow orders strictly".

I don't think that matters I usually use "Coach Follows Depth Chart" medium option. I also notice though that my opponent's coach when they are down by 20+ will keep their starters in even if the score continues into even larger leads.

I actually feel that taking your players off when winning by 20+ points is what screws up my training. I have a good feel for my players and how many minutes they will play under normal situations. So I game plan and set my trainees accordingly...

On the other hand, I don't often know when I'm going to win games by a huge margin. So having all my starters leave the game for a big stretch in the 4th quarter is really annoying.
