It can happen when you select the "coach picks from depth chart" substitution strategy. Please use "strictly follow depth chart" to avoid it.

Excuse me but I really think that I remember having selected that substitution strategy. Have you checked that or are you only guessing that's what I set up?
Thanks in advance, sorry if I am disturbing you unnecesarily, but I really remember to have configured that substitution strategy in order to avoid what happened in the end.

I happened so long ago that I can't even select that lineup in my match setup page already for my incoming games, so I have no other thance than believing you ;=)
