Okay, thank you.
I try to envision the ball somehow being batted up in the air by the opposing team, and then no one gaining possession as both teams keep scrambling for the ball, and then this taking place for as long as the clock continues to count down with the ball finally ending up in the defensive team's hands. It conjures up a fairly comical scene in my head.
Also, I think that technically that would still be a shot clock violation, but I'm not sure.

I'm pretty sure it's a known bug. I think in real basketball it would be a shotclock violation. I think what's going on is that the game engine has decided that a steal is going to happen, but it hasn't figured out when. It's especially frustrating when you are down at the end of the game, your team goes for the steal, succeeds, but it takes so long that the game clock runs out. It really should be that if you don't get the steal in 2-3 seconds, you foul. Just my opinion.

There are instances where the shot clock is at zero, and the team that supposedly has possession of the ball shoots and make the shot.

Oh right, I've seen that, too. I think I've only seen the team in possession make the shot, but never miss.
If I had to guess, I'd say that when this occurs, 90% of the time it's a steal and 10% of the time it's a made FG.

I believe I've seen a team miss the shot after the shot clock doesn't reset, but I'm not 100% sure. Chances are that it probably happened :)

Yeah, that percentage sounds about right.
