The emphasis was on "just a little" more. A better doctor will prevent some injuries and if you get one with massage, your players will stay in shape better if they play a lot of minutes (which your players do). Maybe it's a cheaper investment to do some gameshape management, but in the meantime a massage doctor helps too.
A PR-manager will earn back his money with a little better attendance, and it doesn't even have to be much since he doesn't cost a lot. Besides that, he can help with in game performance if he has a specialty. If you take National Appeal and he's the difference between a close loss and a win on the road, the increased revenue your next home game will more than make up for his salary.

Pr recommended lv 1 except for Poff/out games where you can buy a lv 2 with the speciality you need. Doctor is random so i always had level 1 because when trying the level 4 for three seasons i got the worst injuries of my career so i lost the hope in the doctor value.

Ah ok, So a massage doctor who is compentent would be great for training? Young guys got some young, and got a few thats old.

I'm also looking to switch from going to 6 to 14 players, every 4 or so seasons. Would I need to change my doctor and pr manager every 4 or so seasons?

Generally, competent staff does the job. You can find some with about 8k salary or less, replace after a season or two..
